Preheat oven to 350°F (175°C). Grease a loaf pan or line a baking sheet with parchment paper.
In a large bowl, combine ground beef, breadcrumbs, milk, eggs, chopped onion, minced garlic, salt, and pepper. Mix gently until just combined, being careful not to overmix.
Shape the mixture into a loaf and place it in the prepared loaf pan or on the baking sheet.
In a small bowl, whisk together ketchup, brown sugar, and Worcestershire sauce for the glaze.
Spread half of the glaze over the top of the meatloaf.
Bake for 45 minutes. Remove from oven and spread the remaining glaze over the meatloaf.
Continue baking for another 30 minutes, or until the internal temperature reaches 160°F (71°C) and the meatloaf is cooked through.
Let the meatloaf rest for 10 minutes before slicing and serving.
